Mobile Crisis Peer Case Manager Overview:
The Mobile Crisis Peer Support has experience in the behavioral health care system both personally and professionally with the ability to provide outreach, support and advocacy for those served in the community during a crisis situation.
Qualifications:
Y Bachelor's Degree, Associate's Degree, or college-level academic work in related field strongly preferred; or High School diploma with relevant work experience/knowledge-base of mental health and/or substance use recovery Y Personal experience as a recipient of mental health and/or substance use services with two years of successful recovery, is preferred Y Experience of working with children and youth (up to the age of 21) strongly preferred Y Able to interact professionally and effectively with staff and clients Y Comfortable working in collaboration with local law enforcement and community providers in a crisis situation Y Experience/general understanding of documentation procedures including use of an electronic health record Y Working knowledge of issues facing persons affected by mental illness and chemical dependency Y Familiarity with community supports, as well as local services and systems Y Qualify for and complete New York State Peer Specialist Certification within six months of hire http:

Responsibilities:
Y Provide assistance, support, and advocacy to individuals (and family members) in need of crisis and other mental health services Y Provide follow-up care to community members after provision of mobile crisis stabilization services Y Will assist clinician in the field with assessing barriers to care as well as identifying individual needs to assist with de-escalation on scene.
Peer Support will assist individuals in the community post crisis with securing needed resources such as medical providers, entitlements, food, clothing, shelter and other supports.
This includes the individual and/or family members Y Promote empowerment through meaningful activities and discussions using selective self-disclosure Y Assist individuals in making community connections based on their preferences and needs Y Maintain accurate and up-to-date records and documentation of services as well as any/all interactions with (or on behalf of) clients in our care via an electronic health record Y Communicate effectively both verbally and in written form with community partners Y Treat all clients and staff with dignity, courtesy, and respect Y Individuals in this role will have access to Protected Health Information (PHI) as part of their job duties:
o Access is only allowed on a need to know basis to perform the necessary job duties in this role o Access to the PHI of individuals being served outside of this program is restricted (via the electronic medical record system) o Access to, use of, and disclosure of PHI is to be done in compliance with the agency's HIPAA policy manual Nothing in this job description restricts management's right to assign or reassign duties and responsibilities to this job at any time This position will travel regularly throughout Broome County, so a significant number of work hours may be spent providing services directly in the community.
This position requires the ability to navigate physical space to include stairs.
The position works collaboratively with multiple community partners.
